Consent to the processing of personal data

I, the personal data subject, hereby express my free, unambiguous, and informed consent, in accordance with the requirements of Article 5 of the Law of the Republic of Belarus No. 99-Z dated May 7, 2021, "On the Protection of Personal Data," to Open Joint-Stock Company "Belavia Airlines," Republic of Belarus, 220004, Minsk, 14A Nemiga St. (hereinafter — the Airline), for the processing of my personal data, both using automation tools and without their use, for the following purpose:

Data processing during surveys is carried out using cloud storage. In the event of cross-border data transfer, the Airline guarantees an adequate level of protection of the rights of personal data subjects

By giving this consent, I confirm that the following rights have been explained to me and that I understand them:

  • To receive information regarding the processing of my personal data. You have the right to obtain information about the Airline's location, to receive confirmation of the fact that your personal data is being processed, to learn the specific list of personal data being processed, the source of its receipt, the legal grounds and purposes of processing, the period for which consent has been given, as well as to become acquainted with the name and location of the Airline's authorized persons. In doing so, you are not required to justify your interest in the requested information;
  • To have amendments made to my personal data. If your personal data is incomplete, outdated, or inaccurate, you have the right to submit an application to the Airline justifying the need for such amendments;
  • To receive information about the disclosure of my personal data to third parties. You have the right to receive from the Airline, free of charge, information about the disclosure of your personal data to third parties once per calendar year;
  • To terminate the processing of personal data and/or have it deleted. You have the right to demand, free of charge, that the Airline terminate the processing and/or delete your personal data (provided the Airline has no other grounds for processing as stipulated by law);
  • To withdraw consent to the processing of my personal data. You have the right at any time, without providing an explanation, to withdraw your consent granted to the Airline for the processing of your personal data by: sending an email containing your last name, first name, and patronymic as specified in your questionnaire, along with a statement of your request, to the email address info@belavia.by; or by submitting an application to the Airline (the procedure for submitting an application is described below).
  • To appeal against the actions (inaction) and decisions of the data controller. You have the right to appeal against actions (inaction) and decisions of the Airline that violate your rights in the processing of personal data to the National Personal Data Protection Center of the Republic of Belarus, in accordance with the procedure established by the legislation on appeals of citizens and legal entities.

To exercise the above-mentioned rights, you must submit an application to the Airline in written form or as an electronic document.

A written application must be sent to the following address: Republic of Belarus, 220004, Minsk, 14A Nemiga St.

An application in the form of an electronic document must be sent to the email address info@belavia.by.

In this context, an electronic document means a document in electronic form signed with an electronic digital signature recognized in the Republic of Belarus. The application must contain: last name, first name, patronymic (if any), address of residence (place of stay), date of birth, a statement of the substance of the request, and a personal signature or electronic digital signature.

The period for reviewing your application shall not exceed 15 days. An exception applies to the review of an application regarding the exercise of the right to receive information concerning the processing of your personal data, which will be reviewed within 5 business days.

Please note that upon receiving consent to the processing of your personal data by means of marking (checking) the corresponding box (checkbox) opposite the specific purpose of personal data processing on the website belavia.by, the Airline obtains a lawful basis for processing it for the aforementioned purposes.

Refusal to give consent to the processing of your personal data for the purpose of "Survey of opinions on the quality of services provided by the Airline" will make it impossible to communicate with you on the matters indicated.
